Disasters in Space Flight On January 27, 1967, the three astronauts of the Apollo 4, were doing a test countdown on the launch pad. Gus Grissom was in charge. His crew were Edward H. White, the first American to walk in space, and Roger B. Chaffee, a naval officer going up for the first time. 182 feet below, R.C.A technician Gary Propst was seated in front of a bank of television monitors, listening to the crew radio channel and watching various televisions for important activity. Inside the Apollo 4 there was a metal door with a sharp edge. Each time the door was open and shut, it scraped against an environmental control unit wire. The repeated abrasion had exposed two tiny sections of wire. A spark alone would not cause a fire, but just below the cuts in the cable was a length of aluminum tubing, which took a ninety-degree turn. There were hundreds of these turns in the whole capsule. The aluminum tubing carried a glycol cooling fluid, which is not flammable, but when exposed to air it turns to flammable fumes. The capsule was filled with pure oxygen in an effort to allow the astronauts to work more efficiently. It also turns normally not so flammable items to highly flammable items. Raschel netting that was highly flammable in the pure oxygen environment was near the exposed section of the wires. At 6:31:04 p.m. the Raschel netting burst into an open flame. Uk Student Life

.. was a great way to introduce the various groups that are available to students on campus. The Center for Community Outreach (CCO) includes programs to get students involved in the community through volunteering and educating others about today’s issues. The Student Activities Board (SAB) provides students with events like concerts, cheap movie nights, and homecoming events, which I cannot wait for. Greek life is very large on campus and seems like a lot of work. Intramurals and club sport allows student to stay in athletics even if they’re not Division 1 material. If a student feels overwhelmed by the variety of organizations, the Center for Student Involvement (CSI) helps find an organization that fits them the best. Athletics are a huge part of the University of Kentucky’s identity. Men’s basketball is the most anticipated season. The new head coach, John Calipari, is sure to help out the program and top recruits from around the nation are ready to put in work. The only sports you have to pay to get in are Men’s basketball and football. All the rest of the sporting events are free with student id. Living on campus is not bad at all; there are a variety of different locations to live in and places to eat, but it is better to be closer to all of your classrooms or where have to go often. Being on campus is very safe because UK Alert, the emergency notification system, notifies you about safety issues, and if UK is closed, closing early, or on delay.

Edgar Allan Poe’s ‘The tell-tale heart’ and Ray Bradbury’s ‘The fruit at the bottom of the bowl’ Essay

Compare and contrast the main themes from Edgar Allan Poe’s ‘the tell-tale heart’ and Ray Bradbury’s ‘the fruit at the bottom of the bowl’. The two stories both contain crime, punishment and murder. The two stories both contain two men who become obsessed with either his own or someone else’s body part. They seem very similar but they are actually quite the opposite†¦ The writers both build up tension in there stories they make this clear by adding something about the item in nearly every line, which makes the reader clear of what is going on. Ray Bradbury tries to give us a clue in his title what the story is about, before you have read the story the title is not clear but after you have read the story it becomes clearer, the man in the story murders someone and gets obsessed with cleaning, as he wants to get rid of the evidence. The author uses the metaphor ‘the fruit at the bottom of the bowl’ to emphasise that the man is so obsessed with the evidence and what can be seen he has forgotten about the things that are deeper than the surface what the eye can’t see, like the fruit that is at the bottom of the bowl. In this story we are not given a name, sex or given any kind of information about the character, which is rather strange. Edgar Allan Poe does the same kind of thing with his title ‘the tell-tale heart’ this as well is not clear to us until we have read the story his title explains what happens when the character is caught he/she breaks down in front of two policemen because of his/her heart, his conscious is telling him what to do and finally the character breaks down. The story is about a mad person ‘†¦that I am mad’ who is disturbed by one mans eye and can’t take it no longer and eventually murders the man and gets caught when he/she breaks down in front of two officers, but he/she only thinks they have killed the eye but they don’t realise they have killed the man but deep in there heart they know they have done wrong and their conscious gets the better of them. The fruit at the bottom of the bowl is set at midnight ‘the clock ticked midnight’ and the character who is William Acton becomes obsessed with cleaning as he has killed a man (Huxley) and wants to get purge of the evidence in nearly every line it mentions something about his hands or fingers he is trying to remember what his hands or fingers have touched ‘the fingerprints were every, everywhere!’ at the end of the story it doesn’t tell us if Acton was caught it leaves us to guess what is going to happen to him. As in the tell-tale the character (who we are not told a name or sex) is mad and becomes obsessed with an eye of a man ‘I think it was his eye, yes his eye’ and that’s what causes the murder and we do know that he is caught. They both committed the crime by murder. We do assume that both the main characters are taken away and given prison sentences but it doesn’t actually tell us this is happening because of the way the stories are set out with the flash backs, the past and present tenses we do not know if the story is being told to us even from a prison cell or if they have already received there punishment it leaves us in suspense to what is going to happen to them or what has happened to them. It does give us an idea that they are already sentenced because they are telling the story in the past presents. In the tell-tale heart the narrator is telling the story he or she starts to go mad when he mentions the eye and he starts to build up tension when it comes to the end where the character breaks down in front of the two officers the character breaks down because of the heart beaten in his head but I don’t think he does hear this in his head I think it is his conscious telling him to confess to what he has done in the written story they show this by adding a lot of explanation marks because he is breaking down and it is all happening so fast ‘I could bear those hypocritical smiles no longer! I felt I must scream or die!’ In the two stories the characters choose to kill their victim. In the tell-tale heart the character planned to kill the man he couldn’t take the sight of the eye anymore ‘I made up my mind to take the life of the old man, and thus rid myself of the eye forever’ as in the ‘fruit at the bottom of the bowl’ it was done more spontaneously he just ended up having an argument with a man called Huxley and Acton (the main character) ended up strangling Huxley and killing him. Even though the two stories seem very alike they have their differences. In the ‘fruit at the bottom of the bowl’ the writer uses a range of sentences when he is talking about the past he uses long sentences and in present he uses shorter sentences also the author writes the story after the murder is committed and tells us how the murder is accomplished by using flash backs and includes us into the story when he is talking in the present, which makes the story seem more eerie, as the ‘the tell-tale heart’ is told in a slightly different way it doesn’t use the same style of writing he writes in one format instead of two. He writes in the first person ‘hearken I and observe how healthily- how calmly I can tell you the whole story’. He starts the beginning of the story talking to the reader which drifts you in to the story because the character is not talking sense ‘I heard all thing in heaven and in earth. I heard many things in hell so how then am I mad?’ and you become confused to what is going on and makes you want to read on to find out what the character is saying and what he is on about? In the tell-tale heart the character mentions that he has gone mad ‘why will you say then that I am mad?’ he has lost his mind and I think that is punishment enough for him because he has took away someone’s life now he has had his mind taken from him. In the fruit at the bottom of the bowl I don’t think Acton has gone insane he just becomes obsessed with trying to get disposal of the evidence and this is what makes him become obsessive. I don’t think before the murder either of the characters were mad, maybe the character in the tell-tale heart may have been a bit mentally unstable as it is a bit confusing how he becomes so obsessive over one mans eye but I think that he/she tends to lose their mind after the murder as it has got to him/her a lot. As William Acton also tends to loose his mind towards the end of the story as his hands start to take control ‘but unknown to his eyes, his gloved fingers moved in a little rubbing rhythm on the wall’ and also he starts to talk to himself ‘would u, I would, are you certain, yes’. I think both stories had a well thought out setting and both themes were superior but even though the stories both contain the same contents (murder, crime and punishment) they seem very similar but they are really quite diverse I didn’t realise that until after I had compared the two stories. The two authors use different styles of writing in their stories. I really enjoyed reading the stories but not as much as comparing them and spotting how much they are unlike.

Function Questions on SAT Reading Strategies and Practice SAT / ACT Prep Online Guides and Tips Function questions (also sometimes known as â€Å"meaning in context† questions) make up 17% of all passage-based reading questions, or about 12% of all SAT Critical Reading questions (based on my survey of the four publicly available post-2005 SATs). Answering function questions requires the ability to step back from the text and judge the effect of a phrase in a certain place (as opposed to little picture and vocab in context questions, which are just concerned with meaning). So how are function questions asked on the SAT, and what strategies can you use to answer them? Read on to find out. Note: The advice in this article is still relevant for the new SAT (March 2016 and beyond), but some of the examples have not yet been updated. Function questions ask what a phrase, sentence, paragraph does in the context of a passage. They are often framed as â€Å"The author wrote [this thing] in order to..." Answering these questions can be tricky, because the wording of function questions often makes it seem as if they're asking â€Å"Why did the author do this thing?† Invariably, unless you are the author, your first response will probably be along the lines of "How should I know? I can't read the author's mind!"A better way to approach function questions is to rephrase them from "The author uses the phrase "The Great Old Ones" multiple times in order to..." to something more like â€Å"What effect does mentioning "The Old Ones" multiple times have in the context of [the lines, paragraph, or passage]?† A label sometimes given to function questions is "meaning in context" questions, which I find to be a little too imprecise: the questions are asking about the EFFECT of a phrase in context, not what the phrase means (which would be an inference question). Let's say that I lived in a city that got over 100 inches of snow last winter, and just as I finished clearing snow out of the gutters, it started snowing again, prompting me to sing (sarcastically) "It's the most wonderful time of the year" as I smiled through gritted teeth. The MEANING of the words "most wonderful" in this context would be "worst" (as in "It's the worst time of the year"); the FUNCTION of the words "most wonderful" would be "to suggest that the speaker actually feels the opposite is true." Just as with primary purpose questions, function questions usually have answer choices that are in the form of â€Å"verb a noun† (as in "add gravitas to the mythology" or "convey the passing of time"); the main difference between the two is that function questions ask about relatively small amounts of text (phrases or lines), as opposed to the entire paragraphs covered in big picture questions. In fact, having a good understanding of the big picture/main point of a passage can often assist you in answering function questions. Consider this example: you're asked "The phraseâ€Å"the life-or-death cycle of espionage and counterespionage† primarily serves to..." If you know that the main point is, for instance, about deep sea life, you’re unlikely to assume the phrase â€Å"the life-or-death cycle of espionage and counterespionage† promotes the idea that all underwater creatures are potential spies for other countries. Instead, it's far more likely that the function of the statement is to support the idea that in dangerous environments like the deep ocean, species are constantly evolving to win the fight for survival. Here are a few examples of the most common ways you’ll see function questions presented in the SAT (adapted from actual SAT questions): â€Å"The primary purpose of the statements in lines 36-42 (â€Å"Like†¦from the deep†) is to†¦Ã¢â‚¬  â€Å"In lines 58-73, the description of the ocean floor primarily serves to†¦Ã¢â‚¬  â€Å"The author invokes â€Å"Cthulhu the destroyer† chiefly in order to†¦Ã¢â‚¬  â€Å"The author uses spy imagery in lines 84-85 in order to†¦Ã¢â‚¬  â€Å"Jun asks the questions in lines 41-42 (â€Å"Does†¦frightening†) primarily in order to†¦Ã¢â‚¬  â€Å"In lines 27-30, the author uses the word â€Å"ordinary† several times in order to† Ph'nglui mglw'nafh Cthulhu R'lyeh wgah'nagl fhtagn by Thomas Claveirole, used under CC BY-SA 2.0/Cropped from original. This is not in any way to imply that there is something lurking beneath the surface of the water. Why would you even think such a thing? The Best Strategies for Answering Function Questions Below, I’ve gathered the advice that is most helpful for answering function questions. Some of this advice is specific to certain ways of reading the passage (for instance, advice for students who read the whole passage before answering questions may be different from advice for students who read the questions before looking at the passage), while some advice is useful for everyone. Understand What the Question Is Really Asking I believe that this is the single most important part of consistently answering function questions correctly. The fact that function questions are worded in a way that makes it seem like you need to read the mind of the author (or character) can make these questions seem impenetrable. To some extent, I understand why the College Board words SAT Reading function questions the way it does. Questions that say â€Å"What does the mentioning of two sports stars do† are too open to interpretation, as compared to [actual SAT question] â€Å"The author of Passage 1 mentions two sports stars (lines 31-33) in order to.† Understanding the reasoning behind the wording doesn’t make the wording any less confusing at first glance, however. In order to answer function questions successfully, you must first learn to translate the questions into questions that you can actually answer. Here's an example question: The author most likely refers to the â€Å"flea† in line 4 in order to... Wrong way to rephrase it: "Why does the author refer to â€Å"the flea†?" The answer to this question is going to be something like "Because she feels like it? I dunno." Right way to rephrase it: "What does the reference to â€Å"the flea† DO?" The answer to this question: mentioning â€Å"the flea† emphasizes how good Duke Ellington’s listening skills were (his hearing was so sharp that he could even hear a flea). Corollary: Answer in Your Own Words Coming up with the answer in your own words before looking at the answer choices is just as useful for function questions as it is for many other types of SAT Reading questions. Your answer for â€Å"what does mentioning ‘monsters’ DO?† will probably not be as elegant as the answer choices, but your answer should only include relevant and accurate information, which means matching it to the correct answer should be easier (since the wrong answers may have irrelevant info or interpretations that â€Å"seem like they could be true†). Including irrelevant information in answer choices is a way the SAT will trip you up, particuarly on Reading questions – the College Board knows that you’re used to trying to see things from many angles as part of school (which is ordinarily helpful) and plays to that tendency. Because there's only one right answer on the SAT, if you start with the right answer in your own words it’s a lot easier to choose the right answer in their words. But how do you come up with your own answer for what a phrase, line, or sentence does in a passage in the first place? Look for Context Often, the lines that the question gives you are not necessarily the only lines you’ll need to answer the question - you'll need more context to figure out the function. If you’re struggling with a function questions, the best strategy is to look at the sentences before and after the phrase/sentence/lines you’re given in the question and see if they shed any further light on the phrase/sentence/lines in question. Occasionally, you may need even more context than the surrounding lines to answer function questions (like knowing the bigger picture/main point/perspective of the text/author). If you don’t have that context and are struggling to answer the question, circle it and come back to it after you’ve answered relevant big picture questions (questions about the paragraph/section the lines in question are in, or even questions about the whole passage). This leads nicely into my next point... Answer Questions in the Order That Works for You In contrast to the previous strategies, this strategy is heavily dependent upon which way you read the passage. I present three scenarios below. If you read thoroughly first, then answer questions, you could try answering big picture questions first (they might give you information that's useful for answering function questions), then function and inference questions (answer questions about larger amounts of text while it's still fresh in your mind), and end with little picture, author technique, and analogy questions. Honestly, though, if you read quickly enough to get through the passage and have plenty of time to answer the questions, answering all the questions in order is a viable option: it might be less confusing then jumping all over the questions and decreases the chance you’ll miss one/fill in the answer in the wrong spot. Do some trial and error to see what is most effective for you. If you look at the questions first, then go to the passage, I recommend answering little picture and vocab in context questions first. Answers to these questions will provide more information about the author and/or topic being covered, which can help you with function questions. If you end up needing big picture information in order to answer the function questions, you can always come back to them later (after you've answered the relevant big picture questions). If you skim through first, then read the questions, then back to passage, it's better to answer big picture and little picture questions first and get to function questions later. Why? Unless the phrase, sentence, lines being asked about were in the parts of the passage that you paid close attention to (beginning or end of a paragraph/passage), you probably didn’t get the meaning of the phrase/sentence/lines your first time through. Make the most use of skimming by answering questions that would be better served with a quick skim before answering questions that require a closer look (like function questions). Eliminate Answers The most important thing to remember when answering function questions on SAT Reading is that you must eliminate 3 wrong answers. If you’ve already answered the question in your own words successfully, this step might not even be necessary, because only one answer choices should match. However, the answer choices for function questions are often complex. What happens if your answer doesn't match any of the answer choices, and you have to go through them one by one? At first, it might seem daunting, because how do you figure out if the answer is right? There's so much to go through because the answers are so complicated! Actually, the fact that the answer choices are complex makes everything easier. Why? Because if any part of the answer choice does not match with the passage, then you can eliminate it. Let's look at an example: The author’s use of italics in line 20 serves primarily to (A) draw attention to a commonly known hypothesis (B) stress a speculative aspect of two theories (C) support a difficult claim (D) underscore a surprising point (E) emphasize an area of agreement Unlike with primary purpose questions, the verbs in the answer choices to function questions are usually synonymous (or close to), so you can’t really use that as an area of elimination. Instead, you must focus on the second part of the answer choices. For (B), this would be â€Å"a speculative aspect of two theories.† Are there two theories? If not, ELIMINATE. Is there a speculative aspect to these two theories? If not, ELIMINATE Does the use of italics actually stress the speculative aspect of the two theories? If not, ELIMINATE As you can see, there are many chances for elimination – it should be really hard for an answer to qualify as correct. For this question, the correct answer, (D), passes this test: there is a point, the point is surprising (both theories work, yet both cannot be true), and the italics underscores this (the italics emphasize the fact that both can’t be right). Function Questions: A Walkthrough I'll end this article with a walkthrough of a difficult function question. Here it is: The sentence in which â€Å"difficult† appears (lines 54-55) indicates that the author considers the word to be (A) an exaggeration (B) an estimate (C) an understatement (D) a contradiction (E) a preconception Before I even copy and paste the relevant lines, I’m going to rephrase this question in a way that is answerable: What effect does â€Å"difficult† have in lines 54-55? Okay, now it's time to take a look at those lines. The Tanaina live in an environment that could euphemistically be described as â€Å"difficult.† My thoughts: The word "difficult" appears in quotation marks, which makes me think the author doesn’t mean exactly what is written. If I know the word â€Å"euphemistically† (which I do), this will go a lot smoother. But let’s say for argument’s sake that I don’t know what "euphemistically" means. Hmm. The sentence doesn’t provide enough to answer what â€Å"difficult† does, except that there’s probably a hidden meaning. I need to look for more context. Here’s the sentence from the question again, along with the following sentence (since the sentence is at the beginning of a paragraph, it makes more sense to look at the next sentence, rather than the sentence before it): The Tanaina live in an environment that could euphemistically be described as â€Å"difficult.† Survival, especially in the wild, is always precarious. My thoughts: So the sentence in the question is followed by one that says â€Å"It’s hard to live out in the wild.† Author uses the word â€Å"precarious† which I’ve always associated with things hanging off the edge, as in â€Å"Your glass of seltzer is pretty precarious sitting at the edge of the table there, Laura. You might want to move it† (may or may not be a true story). So if survival is precarious that means it’s like something almost falling over the edge. That’s more than difficult – that’s dangerous. Okay. So let me check the question again: â€Å"The sentence in which â€Å"difficult† appears (lines 54-55) indicates that the author considers the word to be† Right right right. So the situation is actually much worse than just â€Å"difficult† for the Tanaina, and putting quotes around it makes it clear that the author thinks â€Å"difficult† is way too mild a word to describe the situation. So, in my own words, the author considers the word to be too mild. Let’s look at the choices again and eliminate some answers: (A) an exaggeration Nope, â€Å"difficult† is, if anything, the opposite of this. (B) an estimate Not really - this doesn’t make sense. If it were an estimate, there’s no reason for it to be in quotes. (C) an understatement Oh yes. This is the word I was looking for. I’ll just check the other answers†¦ (D) a contradiction No, â€Å"difficult† is not a contradiction, it’s just not extreme enough. (E) a preconception Hmm I guess I could double check to see if the word â€Å"difficult† is used earlier in the passage to describe the Tanaina, although I don’t think it was†¦but wait, no, I already have an answer that is correct. Function Questions: Practice Time! The following four questions are all on the same passage (taken from an official and publicly available SAT). I’ve excerpted only the paragraphs that are relevant for these questions. 1. The author of the passage uses the quotation in lines 5-6 primarily as a (A) vivid expression of how she views words (B) powerful example of what she sought in Shakespeare (C) scholarly citation linking her to poetic words (D) comical introduction to a problem encountered by every dramatic performer (E) pragmatic assessment of the power of words for beginning drama students 2. By presenting both versions of the grandfather’s words (lines 9-10 and lines 15-16), the author primarily conveys the (A) grandfather’s attempts to play with language (B) grandfather’s enthusiasm in spite of her reaction (C) father’s intervention in a private moment (D) ambivalence she feels toward her grandfather (E) significance of the grandfather’s message 3. Answer the question in your own words before looking at the SAT’s answer choices Look for context around the phrases or lines cited in the question to help answer the question. The student will also incorporate local and national health and social policies, including frameworks that are in place in relation to the patients illness. The student identified a patient named Mrs J. She was admitted to hospital due to exacerbation of COPD. Her primary diagnosis is Osteoarthritis of the Hip but also had symptoms of emphysema. Mrs J is a 55 year old woman and is married, Mrs J also has an older child of 34 whom she has become quite dependant on and felt like she had impacted on her childs life and had become a hindrances. Mrs J has become more breathless as her condition develops over time and more so while she was lying in bed unable to carry out her daily activities such as doing the housework, leisure activities, also looking after her appearance as she normally would have done at home . COPD is becoming one of the fastest leading causes of disability (NHS choices, 2008). According to British Lung Foundation, (2010) a recent survey, 83% of COPD patients said their COPD slows them down, 79% said they had to cut down their activities and 56% said their condition has a great affect on their families. COPD is the most common respiratory conditions in adults in the developed world and poses an enormous burden to society both in terms of direct cost to the healthcare services and indirect costs to society through loss of productivity. Recent analysis estimated that National Health Service (NHS) spends  £818 million annually in the United Kingdom (UK). (British Thoracic Society, 2006). However 50% of the cost is accounted for by poorly managed exacerbations resulting in frequent re-admissions to hospital (Coakley Ruston, 2001). COPD is a chronic inflammatory condition of the lungs that causes the respiratory passages to be swollen and irritated, increases the mucus production and damaging the lungs. (Marieb, 2003). COPD develops when irritants are breathed into the airway and down the bronchial tubes. Normally, oxygen is passed through the bronchioles into the alveoli, tiny hollow sack-like structures in the lungs where oxygen is absorbed in to the bloodstream. When air is mixed with smoke or irritants, it will damage the lungs and the ability to take enough oxygen in (NHS-Choices, 2008). Mrs J condition would of been triggered by her heavy smoking, the toxins from her cigarettes has made her bronchioles (airway and lungs) become inflamed and narrowing the airway, this will lead to irreversible damage to the respiratory system by obstructing the bronchial airflow and hindering gaseous exchange within the alveoli (Munden, J, 2007). Mrs J suffers from many symptoms due to her smoking these include shortness of breath, a persistent cough, yellowish green sputum, signs of cyanosis to her lips, also Mrs J has continued to smoke as she thinks the damage has already been done so her condition. The vast majority of COPD patients are smokers. By stopping smoking patients can slow the rate of decline in lung function and thus improve the patients prospects in terms of symptoms and survival. The NICE guidance recommends that short-acting bronchodilators should be used for the initial treatment for breathlessness and exercise limitation and goes on to say that, if this isnt having an effect then the treatment should be intensified using either a long-acting bronchodilator or a combined therapy with a short acting beta2-agonist and a short-acting anticholinergic. The respiratory system is the major part for gases exchange to take place, it allows takes the air that enters are bodies when we inhale and travels through the respiratory system, exchanging oxygen for carbon dioxide and expels carbon dioxide when we exhale (munden, J, 2007). Mrs J has been smoking now for 45 years and on an average day having up to 40 cigarettes a day and is not prepared to quit as she feels the damage is already done. Mrs J smokes for comfort and feels that its all for her pleasure, she has become very isolated, her chronic bronchitis makes her breathless when doing actives and is not able to do her daily activities therefore is becoming depressed. Do this having a huge impact on her mental and social parts of her life. Patients like Mrs J with airflow limitation clinically they have become known as pink puffers and blue bloaters (Kleinschmidt, 2008). Patients with COPD have traditionally been divided into  pink puffers  and  blue bloaters  based on their physiological response to abnormal blood gases.  The former work hard to maintain a normal pO2  which is why they puff away. They tend to have a barrel-shaped, hyper inflated chest and breathe through pursed lips. The latter are blue because of hypoxia and polycythaemia. They are often obese and have water retention. This is why they are bloated. The  blue bloaters  are dependent upon hypoxia for their respiratory drive and to give oxygen and deprive them of this will lead to significant  hypercapnia  and acid base imbalance. (MedicineNet, 1996 2010) Mrs J falls under the term blue bloaters as she linked to chronic bronchitis due to cyanosis which causes a blue tinge to the lips, which occurs from poor gas exchange. Although these conditions separate the patient may present with slight variations of them both, however they do differentiate through their underlying process, signs and symptoms (Bellamy Booker, 2004). The respiratory system can be separated into a conducting and a respiratory portion. The conducting portion consists of the air-transmitting passages of the nose, nasopharynx, larynx, trachea, bronchi and bronchioles. Derrickson   There are two separate arterial systems in the lungs, the pulmonary arteries which carry deoxygenated blood, follow the respiratory passages and end in capillaries in alveolar walls, and the bronchial arteries which carry oxygenated blood. The lungs are richly supplied with lymphatic vessels, which are organized into two sets a deep set that accompanies the pulmonary vessels and airways and a superficial set that lies beneath the visceral and parietal pleura. Little or no anatomises occurs between the two plexus except in the region of the hilum. (SUNY Downstate Medical Center, 2008) The lung is the organ for gas exchange; it transfers oxygen from the air into the blood and carbon dioxide from the blood into the air. To accomplish gas exchange the lung has two components, airways and alveoli. The airways are branching, tubular passages like the branches of a tree that allow air to move in and out of the lungs. The wider segments of the airways are the trachea and the two bronchi. The smaller segments are called bronchioles. At the ends of the bronchioles are the alveoli, thin-walled sacs. With in the Lining the of the nasal cavity is a mucous membrane full of blood vessels, with the many blood vessels this enable the nose to warm and humidify the incoming air quickly. Cells in the mucous membrane produce mucus and have tiny hair like projections (cilia). Usually, the mucus traps incoming dirt particles, which are then moved by the cilia toward the front of the nose or down the throat to be removed from the airway. This action helps clean the air before it goes to the lungs. Ciliated Columnar Epithelium which contains goblet cells and cilia, the goblet cells are responsible for secreting mucus which is able to trap the finer particles from inspired air and the cilia which are fine hairs to trap particles from going into the respiratory track. The cilia carrys the particles by a sweeping the mouth or nose where it can then be swallowed, coughed or sneezed out of the body (Munden, 2007). The two major sources of mucus secretion in the respiratory tract are the surface epithelial goblet cells and mucous cells. In lungs, goblet cells are present in the large bronchi, becoming increasingly thin toward the bronchioles. The sub mucosal glands are in the airways with the thickness decreasing with airway quality. With COPD, sub mucosal glands increase in size (hypertrophy), and the number of goblet cells will increased (hyperplasia) (Rogers, Jackson, 2001). The increased of goblet cells to ciliated cells will cause thickness in the bronchioles, this then impairs clearance of mucus. Patients affected by COPD also show the presence of edema, which can further reduce airway size and compromise the lung function. (Postma and Kerstjens, 1998). In clinical studies, these inflammatory parameters have been shown to correlate with a reduction lung function and an exaggerated bronchoconstriction (Postma and Kerstjens, 1998). The goblet cells are replaced within the small airways (bronchi) with Clara cells they are another form of secreting cell that are form ciliated cells and to help renew the bronchiolar epithelium, they produce hypophase component and a protease inhibitor these help protect the lungs by clearing foreign bodys from the airway (Stokley et al, 2006) Gasses exchange in the lung takes places in the airway with in the alveoli, The largest airway is the windpipe (trachea), this then branches into two smaller airways; the left and right bronchi, At the end of each bronchiole are thousands of small air sacs (alveoli) the walls are a thick network of tiny blood vessels called capillaries. The thin barrier between air and capillaries allows oxygen to move from the alveoli to the blood and allows carbon dioxide to move from the blood to the capillaries into the alveoli (Matterporth Matfin, 2009). Mr J has a poor respiratory drive due to having poor ventilation on the lungs. Hypoxemia and hypercapnia can take place due to the poor ventilation, hypoxia in Patients with COPD like Mrs J have raised CO2  levels and  depend  on a deficiency of oxygen (hypoxia) to encourage respiration. They will develop increased CO2  retention,  respiratory acidosis  and subsequently will require mechanical ventilation (oxygen). It also causes the kidneys to produce erthpoietin which stimulates excessive red blood cell production as Mrs J has poor ventilation intake the blood cells are not fully oxygenated there for cyanosis can occurs (Munden, J, 2007).This has lead to hypercapnia due to raised carbon dioxide levels resulting from suppression of hypoxic ventilation drive. However, this understanding does not account for the many factors that contribute to the control of breathing in patients, and has resulted in oxygen being withheld inappropriately from some patients with acute respi ratory failure.   (Brooker Nicol, 2004). Mrs J has lent to live in hypoxic drive as she has made adjustments in her life and has lived like this for 4 years now and has become normal to her. Mrs J has sensitivity to falling oxygen levels rather than raised carbon dioxide (Brooker Nicol, 2004). A higher level of oxygen within Mrs Js system reduces the stimulus to breath therefore inducing carbon dioxide maintenance (Walsh Crumbie, 2007). Emphysema occurs when the air sacs at the ends of your bronchioles gradually gets destroyed, smoking is the leading cause of emphysema. Resulting in the air sacs (alveoli), in the lungs becoming over stretched making their thin walls tear, there for losing the lung tissue and elasticity. The lungs cannot expand or contract fully, and so become less efficient when breathing. (McCance Huether, 2006)   As the condition worsens, emphysema turns the air sacs which are like a bunch of grapes to become enlarged, irregular pockets with gaping holes in their inner walls; this reduces the number of air sacs and keeps some oxygen entering the lungs from reaching the bloodstream (Munden, J, 2007). This makes you try and breathe harder as insufficient oxygen is not getting into our airways. Our bodies compensates by lowering cardiac output and we then begin to hyperventilate, which then results in limited blood flow through fairly well oxygenated lungs this is in contrast to chronic bronchitis (Brooker Nicol, 2004). Due to low cardiac output in Mrs Js body it will tend to suffer from tissue hypoxia leading to weight loss or muscle wasting (McClance Huether, 2006). Mrs Js chronic bronchitis is a Type 2 respiratory failure; this refers to hypercapnoea, which is a presence of an abnormally high level of carbon dioxide in the blood steam, which can occur with or without hypoxia. This type of respiratory failure is caused by a decline the amount of gas inhaled and exhaled (Higgins,D., Guest,J, 2008). Respiratory failure occurs when alveolar ventilation is ineffective to expel carbon dioxide in the body. Poor ventilation is due to reduced ventilation effort, it affects the lung as a whole, there for carbon dioxide accumulates in the lungs this could deadly if it is not treated (Partridge, 2006). This process is seen in patients such as Mrs J with COPD and can be made more aggressive by an illness (Higgins,D., Guest,J, 2008). Mrs J views her quality of life as poor and hates getting up in the mornings, this is because Mrs J becoming breathless and not being able to fulfil her daily activities. COPD can leave patients feeling anxious and suffering from panic attacks (Christen Antoni, 2000). Mrs J feels frightened becoming breathless as she has had frequent admissions of exacerbation with long stays in hospital, exacerbations is a increased case of breathlessness and sputum experience and very distressing for patients and disruptive to their lives (Alaxander,Fawcett Runciman, 2006). Because of Mrs J being short of breath she feels very lonely and isolated, Mrs J has now got a low self esteem and self confidence this has been induced by her affects of the disease, any activities such as washing, dressing and shopping has become quite dependant on her family in these tasks and feels helpless and a burden to them. The psychological affects of living with COPD can be overwhelming the anxiety and panic levels are very high as Mrs J is living in constant fear and frustration each day of the affects of the illness. COPD patients tend to have a high prevalence rate of clinical depression (N, Haynes, 2000). Depression is very common in COPD patients, around 40% are affected by severe depressive symptoms or clinical depression (Henriksen,A, 2008). Since Mrs J has been diagnosed she has had to make some psychological adjustments, she has some challenges ahead of her such as maintaining her ADLs i.e. self image and personal hygiene. Mrs J feels weak and drained on any activities she does due to breathlessness, most of the time she has to rely on her husband and family to meet these needs and has become dependant upon them; this can lead to activation of negative self image which has lasting changes to the patient (Christen Antoni, 2000). Mrs J feels that dealing with her chronic illness she has had to challenge her own self worth, her own sense of vulnerability and to think of what the future has in hold for her (N, Haynes, 2000). COPD may also adversely affect Mrs J self image which is negatively stigmatised her self inflicted this illness on her self due to smoking (Gore et al, 2000). Stigma has had a huge impact on Mrs Js low self esteem as she has smoked most of her life and has caused her condition to progress, so she only has herself to blame. Having low self esteem can be very difficult for some patients to handle as is plays a role in their lives (Christen Antoni, 2000). The more inactive Mrs J will becomes it will decrease her self esteem and can have an impact on her well being (Haynes, 2000). Mrs J also thinks that she has become depressed due to her condition which has had a huge impact on her quality of life with has lead to isolation and dependant on family and friends (Barnett, 2008). COPD patients are being stigmatized in todays society, this disabling condition has been linked to poor lifestyle changes and people that have smoked (Bartolame, Berger, 2009). Patients with COPD feel stigmatised due to medical profession as they have been labelled under pink puffers and blue bloaters this can have a negative impact on their image (Johnson et al, 2007). Mrs J feels that the main reason that she has become isolated is that she is embarrassed by her visible side affect due to poor mobility and breathlessness. COPD research has mainly focused on the medical side rather than the social impact of the disease. This neglect is seen as patients are not prepared for wider social attitudes of smoking related to their illness and has been looked at as self infliction and avoidable conditions creating stigma which is attached to the disease (Johnson et al, 2007). The National Clinical Strategy for COPD is currently being developed by the Department of Health. This was previously known as the National Service Framework (NSF). Following the Darzi report which stated that high quality for all this was based upon ways to reshape and improve the NHS. The National Clinical Strategy will address the lack of awareness of COPD and also focus on the undiagnosed or inaccurately and also making clear pathways for those diagnosed. (NICE, 2004). NICE guidelines on COPD Cover a full range of care that should be available from the NHS to adults like Mrs J who has this illness and to have the appropriate support and treatments this includes smoking cessation, pulmonary rehabilitation and management of exacerbations and to have easy access to these services (NICE, 2008). COPD has had a huge negative impact on Mrs Js life she has had to overcome challenges due to her COPD such as her basic day to day activities, by making some adjustments to meet these tasks. With Mrs J disabling disease this has lead to loss of function, low self confidence and has become socially isolated in her own home due to her clinical depression (Brooker, 2005). Alexander, M. MEGAN’S LAW PROTECT THE CHILDREN OR THE PEDOPHILES Megan Kanka was an innocent little girl, someone’s daughter, sister, and best friend. The defendant, Jesse Timmendequas, changed all of that. He changed it brutally, savagely, and permanently. In a few moments of unspeakable horror, the defendant destroyed all of Megan’s dreams, all of that joy, all that hope, all that promise. In those few moments, he destroyed Megan Kanka’s life. She would never live to see her wedding day, never have children, and never embrace her family again. Jesse Timmendequas took Megan’s life on July 24, 1994. Her funeral was held on Wednesday, August 3, 1994. Jesse Timmendequas was a twice convicted sex offender. He moved in across the street from Megan’s home without the family’s knowledge of his history as a pedophile. Joseph Cifelli was another convicted sex offender who had spent nine years at the Adult Diagnostic and Treatment Center. He had been convicted after admitting to repeated sexual abuse of a relative that had begun when the child was nine. A third man, Brian R. Jenin had been convicted twice of crimes involving young boys. Jenin, along with Timmendequas, was under investigation in connection with the unsolved murders of two boys in San Diego. All three were Megan's neighbors. Jesse Timmendequas’ convictions stemmed from a 1981 attack on a 5 year old girl, for which he served 9 months. That same year he was convicted for an attempted rape of a 7 year old girl, for which he was sentenced to ten years. After serving only six years, he was free to lure 7 year old Megan into his home where he brutally raped and strangling her with a belt as she bit and fought for life. He knocked her to the floor, hitting her several times in the head. He wrapped her head in plastic shopping bags to prevent her blood from staining the rugs. He then took a toy box and stuffed her inside. Megan’s body was found in a weeded area of a nearby park near a portable toilet. On May 30, 1997, a jury returned a guilty verdict on all counts of murder including capital murder, kidnapping, and aggravated sexual assault and sentenced Jesse Timmendequas to death. It was this little girl’s brutal death that prompted her parents, Richard and Maureen Kanka, to fight for broad based community notification. Megan’s parents believe that if they had known that a pedophile lived nearby, this heinous crime... ...s due process. Due process is best defined in one word--fairness. When a person is treated unfairly by the government, including the courts, he is said to have been deprived of or denied due process. The most common and successful due process challenge is procedural due process. This challenge has occurred mainly in states that classify offenders according to the level of risk they pose to communities. This argument centers on whether an offender is allowed a hearing to challenge a risk classification. The Massachusetts Supreme Court recently found that provisions of the state’s community notification law violated constitutional due process Doe v. Sex Offender Registry Board, Mass. SJC-07608 (July 24,1998). The United States Supreme Court has yet to weigh in it’s opinion as to whether or not offender’s due process or Ex Post Facto rights are being violated by Megan’s Law. Are we bothered by the fact that in the current social climate, the rights of convicted pedophiles are routinely violated and nobody cares. Rules of evidence are stretched, and terms of punishment are increased. The danger of this precedent is impossible to ignore. As parents, though... are we okay with it.
